Nurse (Facility Telehealth Coordinator)

Duties and responsibilities include, but are not limited to, the following:
Supervisory oversight of all Telehealth programs :
Home Telehealth, CVT, Store Forward. Provide strategic management and direction for the delivery of quality care to Veteran patients. Plan, organize, direct, and evaluate telehealth programs using measurable objectives and outcomes. Assure telehealth activities are integrated with overall organizational goals and policy development. Provide programmatic oversight and direction to assigned care areas, including but not limited to:
staffing (includes full leave granting authority); space; quality improvement; resource management; coordination of care with other disciplines. Serve as a resource regarding complex patient needs assuring coordination efforts are implemented with other services to address patients' needs in a manner that is seamless to the patient. Provide guidance and oversight to assure the development, implementation, and maintenance of current and evidence based standards of nursing practice Conduct clinical and environmental rounds in order to evaluate organizational care delivery identifying opportunities for program improvement and effectiveness and to assess the therapeutic milieu and safety in clinical areas. Guide and assure the integration of nursing and other care activities as part of an interdisciplinary team approach to achieve organizational goals for quality patient care services across the continuum of care, i.e., inpatient, specialty care, outpatient, and extended care in the Telehealth Program. Participate in policy development related to and implementing standards of professional nursing practice. Participate in contract process including but not limited to determining need, work statement preparation, administration, and oversight of contract clinics. Communicate and interact appropriately and courteously with internal and external customers. Maintain confidentiality of written, electronic, and verbal patient/employee information. Work and function in a safe manner including infection control and operation of equipment. Assure patient safety is paramount in all care provided. Integrate and apply pertinent aspects of VHA, VISN, and Medical Center performance improvement program into his/her professional practice. Utilize evidence based practice, nursing research, and current literature to assess, develop, and implement nursing care models and delivery systems. Assure that care is provided as appropriate to the age specific needs of the adult and geriatric patient. Assure compliance with accrediting, practice, and regulatory requirements. Proactive in identifying potential crisis situations and responds effectively in emergency situations. Facilitate referrals and/or consultations to achieve the multidisciplinary plan of care. Function as a consult person to physicians, nursing and other personnel Monitors staffing needs and maintains adequate levels to provide safe patient care. Maintain a safe work environment by routinely inspecting the area and requesting engineering/housekeeping assistance as needed. Provide timely follow-up on all patient complaints/concerns. Communicate and/or mediate effectively with staff and support personnel to solve patient care and staff issues. Requests supplies and equipment as needed. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Experience:
Telehealth experience; one (1) or more years of supervisory experience. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. The incumbent must be able to perform all activities of the position without restriction. The position involves a considerable amount of computer and telephone work while sitting or standing. The position requires the nurse to work for periods of time with arms and hands at a computer and on the telephone. The position requires visual and auditory acuity at a level to provide safe and effective nursing care. The position requires a considerable amount of sitting during patient consultation and typing when entering clinical notes, looking up lab results and other information. This position requires walking the facility buildings and services. Active and valid drivers license as this position will require travel to the Community Based Outpatient Clinics as needed.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
