Occupational Therapist Home Based Primary Care

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until April 06, 2021. The initial cut-off date for referral of eligible applications will be November 17th, 2020, with subsequent cut-off dates on the 1st of each month. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until positions are filled. The Staff Occupational Therapist performs occupational therapy evaluation and treatments with male and female, ages ranging from young adult to geriatric, having a variety of disabilities. These cases may be relatively complex in nature with chronic and acute disease entities including but not limited to central nervous system disorders, joint diseases, circulatory and pulmonary diseases, diabetes mellitus, altered thought processes, amputations and multisystem disease combinations. This position primary responsibility is for providing in-home occupational therapy assessments, and the planning, administration and management of rehabilitation in the home for patients of the VASLCHCS Home Based Primary Care (HBPC) Program. The OT will provide these in-home services to patients with a wide variety of multiple, complex chronic illnesses and/or profound disabilities. The population served is veterans enrolled in the VASLCHCS from all age groups who would otherwise receive their medical care at the medical center. The HBPC occupational therapist is guided by HBPC Policies and Procedures and by the standards of the American Occupational Therapy Association and the State Division of Occupational and Professional Licensing. Serves as a liaison for Occupational Therapy and the HBPC program and functions as a member of the HBPC multidisciplinary specialty care team and other patient care teams as needed to provide quality, effective, and efficient patient centered care. This position exists at the VASLCHCS and involves regular independent travel to patients' homes within identified HBPC service area. Travel may exceed SO miles round trip. The Occupational Therapist, HBPC position, is aligned within the PM&R Service. Performs occupational therapy evaluation and treatments primarily with patients, male and female, ages ranging from adult to geriatric, having a variety of disabilities, These cases may be relatively complex in nature with chronic and acute disease entities including but not limited to central nervous system disorders, joint diseases, soft tissue disorders, circulatory and pulmonary diseases, diabetes mellitus, altered thought processes and multisystem disease combinations. Maintains a level of professional knowledge appropriate for age, sex and ethnic group of patients seen by the OT service. Receives patient referrals from appropriate consult sources and prioritizes patients for evaluation and treatment in a timely fashion. Reviews charts to assess appropriateness of the referrals and possible contraindications for treatment. Responds to patients, families, visitors, and co-worker's requests for assistance in a prompt and friendly manner. The primary age of patients treated and cared for are normally at the geriatric level, i.e. 60 years of age or older, although occasionally there may be younger patients between the ages of 18-60 years of age that require care. The position requires the incumbent to possess or develop an understanding of the particular needs of these types of patients. Sensitivity to the special needs of all patients in respect to age, developmental requirements, and culturally related factors must be consistently achieved. Bachelor's degree in occupational therapy and two (2) years of experience as an occupational therapist; NOTE:
The baccalaureate degree must be from an approved program prior to the AOTA January 1, 2005 decision that the Accreditation Council for Occupational Therapy Education (ACOTE) would only accredit master or doctoral degree programs in occupational therapy. OR Bachelor's degree in occupational therapy and two (2) full years of graduate education in a related field; NOTE:
The baccalaureate degree must be from an approved program prior to the AOTA January 1, 2005 decision that ACOTE would only accredit master or doctoral degree programs in occupational therapy. OR Master's Degree or higher in occupational therapy NOTE:
Individuals must be a graduate of a degree program in occupational therapy approved by the ACOTE or predecessor organizations. Certification. Candidates must possess a current NBCOT certification as an OT. State Licensure. Candidates must possess a full, current, and unrestricted state license to practice occupational therapy in a state, territory or Commonwealth of the United States (i.e., Puerto Rico), or in the District of Columbia. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Preferred experience in home health setting. Grade Determinations:
Occupational Therapist, GS-12 Education, Experience, and Licensure. Completion of one year of experience equivalent to at the GS-11 grade level and directly related to the position being filled. AND Must meet the Demonstrated KSAs. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Knowledge of occupational therapy principles and techniques consistent with current clinical standards based on OT theory and evidence based practice. Knowledge is inclusive of physical, occupational, cognitive, and psychosocial functional deficits. Ability to collaborate and communicate orally and in writing with all internal and external stakeholders. Ability to use critical analysis, clinical reasoning, and creativity to independently solve complex problems related to adapting and modifying assessments, treatment plans, activities and procedures to meet the needs of patients. Skill in procuring, fabricating, adjusting, adapting, and modifying orthoses, splints, and adaptive equipment for activities of daily living (inclusive of durable medical equipment). Ability to conduct OT related in-service and clinical training. Assignment. OTs at this level practice independently and are responsible for comprehensive assessment of functional and occupational roles using standardized and non-standardized evaluation tools. OTs select and provide direct occupational therapy interventions and procedures with varying degrees of complexity to enhance safety, wellness, and performance in ADL, instrumental IADL, education, work, play, leisure, and social participation. OTs demonstrate a full professional understanding of the theories of treatment in occupational therapy and their proper application through the use of activity analysis, behavioral intervention, frame of references, and therapeutic procedures. They are assigned to all program areas within VHA and provide professional, independent occupational therapy services. OTs at this level may be given assignments in any area where advanced specialized knowledge is not required on a recurring basis due to the patient population served and complexity of the facility and/or treatment area. Individuals may perform ancillary assignments, including program management duties on an occasional basis, where the complexity and amount of work is not substantial (less than 25% of the duty time). The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
