Responsibilities:
In home skilled nursing visits, initial and ongoing assessments Monitor and report changes in patient symptoms or behavior Communicate with collaborating physicians or specialists regarding patient care Maintain accurate patient medical records and plan of care Provide compassionate emotional support to patients and their family members Communicate effectively to interdisciplinary team members Observes/ensures compliance with Medicare and State guidelines
Qualifications:
Previous experience in Home Health and Hospice preferred Ability to build rapport with patients, families, and facilities Strong problem solving and critical thinking skills Ability to thrive in a fast-paced environment Job Type:
Part-time Pay:
$25,000.
